Description:
LSSM is a multi-faceted service organization of the Evangelical Luther Church in America. It responds to the needs of people thorugh a wide range of services including housing,outreach, child and families, aging and persons with disabilities. Our primary goal is to serve those who are most vulnerable which has been interpreted as individuals with limited or no resources as well as those without family or basic support systems.
History:
MapleCreek is comprised of three centers. The Terraces is a senior living center. The Lodge is a 125 bed skilled nursing home, including a specialized memory care unit. The Woods is our memory care center. All are located on a beautiful 40 acre campus in southeast Grand Rapids. MapleCreek began in 1924 as the Lutheran Home for the Aged in a building on Division Street. As time went by, this facility grew into a 60 bed basic nursing facility with an attached 16 bed Home for Aged. In 1976, the 32nd Street property was purchased and The Terraces was opened. In that year, a plan for the provision of a continuum of care on one campus was developed. The Lodge was opened in January of 1987, providing care to the elderly who needed skilled nursing care, as well as intensive rehabilitation for developmentally disabled adults. In 1993 the program for the Developmentally Disabled was closed as all the clients were able to be placed in group homes. In 1994, the Harbor, a specialized memory care unit was opened. In 1994, The Woods opened, thus providing a link in the spectrum of care offered at the complex by providing care for those persons needing more assistance than that offered by the Terraces but not needing skilled nursing services. In 1996, MapleCreek at Home began providing in home health services to selected elderly clients and persons with disabilities. “The Neighborhoods” were added to the community in 1998. These two-15 bed units based at The Terraces provided a more structured environment for seniors with early to mid-stage Alzheimer’s disease. In the spring of 2007 we started a major expansion and renovation that will allow us to better serve the elders in the Grand Rapids area today and in the foreseeable future. The Woods is now a Memory Care Unit and some assisted living for 60 people. The Lodge will continue to host 125 residents, however the interior has been updated and an extensive therapy wing added. The Terraces has become a 160 unit apartment complex for independent older adults with some a la carte services available. An addition of 40 condominium style units will provide totally independent living – The Cottages.
