The mission of Care Alliance is:to provide high-quality, comprehensive medical and dental care, patient advocacy, and related services to people who need them most, regardless of their ability to pay.
Description:
Care Alliance is a nonprofit, Federally Qualified Health Center that provides primary health care, dental care and supportive services to individuals and families experiencing homelessness, living in public housing, uninsured or underinsured. Care Alliance provides primary health care and comprehensive oral health services; HIV testing, treatment and counseling; diabetes management; case management; short-term substance abuse and behavioral health counseling; outreach to people living on the street; and women’s health services. Care Alliance has three primary sites; one strategically located near the two largest homeless shelters in downtown Cleveland, and the other two located within public housing facilities on the near east and west sides of the city. In addition to its three main locations, Care Alliance serves patients at outreach sites which include homeless shelters, drop-in mental health centers and AIDS service organizations (ASO) throughout the City of Cleveland. In 2009, Care Alliance served 9,000 patients and generated nearly 30,000 encounters. Of the patients served, seventy percent were homeless, ninety-four percent were living below 100% of the federal poverty line and eight-seven percent had no form of health insurance.
History:
Care Alliance Health Center was established in 1985 as one of the original Robert Wood Johnson Foundation Health Care for the Homeless projects in the nation; and in 1993, became an independent nonprofit organization. In 1998, Care Alliance assumed the responsibility of providing health care for residents living in public housing, becoming the local grantee for the Department of Health & Human Services’ Public Housing Primary Care Program. In 2000, Care Alliance was awarded a Ryan White Part C (Title III) grant to provide primary health care and case management services to persons living with HIV/AIDS; and in 2002, in order to begin to address one of the largest unmet health care needs in the State of Ohio; Care Alliance began its comprehensive dental program. Today, Care Alliance is a well-respected and integral member of the local service delivery system of safety-net providers with over twenty years of experience providing high quality, comprehensive health and dental services to individuals and families who are homeless, low income and uninsured.
